Rebecca Lovely Outstanding Service Award Nomination Form

The Rebecca Lovely Outstanding Service Award recognizes exemplary performance of administrative staff identified by their faculty and peers as having consistently excelled in their position and demonstrated integrity and a strong commitment to the mission and values of the Department of Anesthesiology and the University of Florida.

The deadline to submit your nomination for this year’s Rebecca Lovely Outstanding Service Award is April 03, 2026 at 5 p.m.

In addition to the Rebecca Lovely Outstanding Service Award, we will also be giving out some additional awards based off of these nominations: Leadership in Patient Care, Excellence in Research, Outstanding Educational Support, and Cultural Catalyst.

Award Information and Criteria

Eligibility

Any Department of Anesthesiology administrative staff member in a full-time TEAMS position and currently employed is eligible for nomination (see attached list).

Nominations

  • Nominations should be based on contributions from April 1, 2025, through March 31, 2026.
  • Nominations should be submitted using the form above.
  • Nominations must be submitted prior to the deadline.

Nominations must include the following information:

  • Statement describing the nominees’ exceptional contribution to the department.
  • Detailed and specific description of what makes the nominee outstanding. Explain how nominee meets or exceeds the ideals identified in the selection criteria below.
  • Contact information for the nominator and the nominee.

Selection Criteria

Award recipients will have made a significant impact in the department through outstanding teamwork, integrity, accountability, effective communication, and innovation by one or more of the categories:

  1. Rebecca Lovely Outstanding Service Award – Honors administrative staff exemplifying integrity, consistent excellence, and commitment to our missions, celebrated annually in Rebecca Y. Lovely’s honor.
  2. Leadership in Patient Care Award – Honors exemplary leaders who improve patient outcomes and safety, innovate care processes, and inspire teams to deliver exceptional care.
  3. Excellence in Research Award – Honors researchers advancing medical knowledge, improving patient care, strengthening departmental research, and demonstrating rigor, mentorship, and impactful results.
  4. Outstanding Educational Support Award – Honors individuals enhancing learning through innovative programs and support, improving outcomes, fostering excellence, and preparing future professionals.
  5. Cultural Catalyst Award – Honors individuals who foster a positive, supportive culture, uplifting colleagues, promoting teamwork, and inspiring a valued, empowered, vibrant workplace.

For all categories, recipients will have demonstrated the following:

  • Consistently and substantially exceeds the expectations of the position, performing at a level above and beyond normal job requirements and expectations; has made important and significant contributions to their area; contributes and demonstrates commitment to the mission and core values of the department and university.
  • Consistently and substantially demonstrates an ability and willingness to work positively, respectfully, and effectively with others; demonstrates ability to foster collaboration, communication and cooperation among colleagues and members of the campus community.
  • Significantly improved a work process or system or has significantly increased the efficiency of an operation in the department; seeks to improve the quality of work assigned; demonstrates effort to expand work responsibilities.

Selection Committee

All nominations will be presented to the Leadership Team who will serve as the Selection Committee. The Selection Committee will review nominations and select one winner per category.

Nomination Deadline

All nominations must be submitted to the Selection Committee no later than Friday, April 3, 2026.

Description of Awards

  • One individual will be selected per award category.
  • Winners will receive a plaque.
  • Winners will be announced and presented with awards at the Award Ceremony on Friday, May 1, 2026.
